What Do We Provide?

Registered Nurses

A Registered Nurse will meet with you upon admission and approximately every 90 days. You will also have 24/7 access to an RN should you have any nursing questions that arise.  The Registered Nurse will create a care plan that is personalized to your needs, and will be used by the CNA and Homemaker to guide their care.

 

Skilled Services

 

As part of your nursing assessments, if the RN feels you may benefit from skilled care such as Physical Therapy, Occupational Therapy, or more frequent Nursing visits we will request an order from your physician.  You may also call and request these services if you or your loved one feel they may be of benefit to you.

 

Certified Nursing Assistants

 

Our CNAs provide personal care services such as bathing, dressing, grooming, feeding, medication reminders and assistance with ambulation and transfers, as well as meal preparation, as outlined in the client's care plan.

 

Certified Homemakers

 

Our Homemakers complete a 25 hour, state-approved, homemaker training course.  They are an integral part of our team and assist with light housekeeping such as laundry, dusting, vacuuming, meal preparation and floor care.  In addition, they provide tranportation for banking, grocery shopping and errands as well as providing companionship and safety.

 

Case Management

 

Case management can arrange transportation to doctor's appointments, order medical equipment and supplies, arrange for your medications to be packaged and delivered and help coordinate your care among your health care providers.
